The Triad Community Board assists in overseeing the delivery of mental health, substance use and intellectual/developmental disability benefits in the Cardinal Innovations Triad service area (Forsyth, Davie, Stokes and Rockingham Counties).
The Triad Community Board is comprised of three (3) members from each county within the Triad service area. The membership includes a county commissioner or designee; a client or family member; and another citizen stakeholder. One member of the Consumer and Family Advisory Committee (CFAC) represents the Triad area on the Community Board. Members shall serve for a term of three (3) years.
The Triad Community Board meets four (4) times annually at the Triad Community Office, 4045 University Parkway, Building 4045, Winston-Salem, NC 27106 at 5:30 p.m.
