Because Forsyth County is committed to helping retirees take charge of their health, the County offers a voluntary, free and confidential Wellness Screening and Health Risk Assessment (HRA) for retirees and their spouses who participate in the County’s health insurance.

The Wellness Screening includes:

  • Height, weight and blood pressure
  • A blood test to determine risk factors for heart disease, stroke, and diabetes

The Health Risk Assessment is a health survey that is completed online. After you complete the Health Risk Assessment, you will receive an easy-to-read, comprehensive report with your laboratory results, descriptions of tests, and healthy tips. This information will be mailed to your home address and your individual results will not be shared with the County.

Retirees and their spouses who complete both the Wellness Screening and Health Risk Assessment (HRA) will be eligible to receive a discount to their monthly health insurance premium once the screening process has been finalized.

Enroll in the program in three easy steps:

  • Step 1: Complete an HRA (now through April 12)
  • Step 2: Schedule a Biometric Screening Appointment
  • Step 3: Complete the Biometric Screening (April 17-21 at the Government Center from 8:30 am- 1:30 pm or April 24, 25 from 2:00 pm- 7:00 pm and 26, 27, 28 from 7:00 am- 12:00 pm at Public Health)

Your confidentiality is assured. All information gathered in the screening and the laboratory report is personal and confidential, as protected by federal law. Forsyth County will not have access to your individual results. We value your privacy and understand its importance. There are many steps in place to assure confidentiality.
